This form should be used by instrument holders to notify the department of the operator as required under the Petroleum and Geothermal Energy Resources (Environment) Regulations 2012, Petroleum (Submerged Lands) (Environment) Regulations 2012 and Petroleum Pipelines (Environment) Regulations 2012.

To be completed after the submission of an Environmental Plan, this form is to be completed by operators. It informs the Department of Mines, Petroleum and Exploration of the instruments to be used, the instrument holder(s), the details of the activity and to provide the endorsement of the instrument holder(s).
