To register a new Public Interest Disclosure (PID) officer or remove an existing one, please complete the PID Declaration Form.
Seek approval from your Principal Executive Officer before completing the Public Interest Disclosure Officer Declaration.
Complete the declaration form and have it authorised by your Principal Executive Officer.
