Privacy Collection Notice
The Department of Local Government, Industry Regulation and Safety (LGIRS) collects personal information for the purpose of:
- regulating the electrical industry in Western Australia;
- processing your application;
- administering licences and registrations;
- compliance and enforcement activities;
- engagement and educational activities;
- other regulatory functions.
Your personal information is collected under the relevant provisions of the Electricity Act 1945.
If you do not provide the requested information, LGIRS may be unable to process your application or provide the relevant services.
Your information may be disclosed where authorised or required by law. This may include an online licence search, a publicly available register, to other government agencies, regulatory bodies, emergency services, and other third parties.

Change of name
This form is used to notify the department of a change of name to an electrical worker's licence, permit, or certificate of registration and obtain an updated licence card.

There is no fee payable to change the name on your electrical licence or permit.
