Before you apply
1. You'll need
- A copy of your X-ray Licence (if working on X-ray Equipment).
- A copy of your professional or trade qualifications.
- A copy of your relevant Certificates for Imaging Equipment.
- A list of the type of imaging equipment used during your training, issued by your Employer on Company Letterhead.
Note: Above evidence not required when re-applying with same Employer
2. Additional documents
You'll also need:
- Proof of ID ready to upload.
- A police check ready to upload (dated within the past 3 months).
- Visa/Mastercard for fee payment.
3. Complete application form
Print or digitally complete the relevant application form.
