The Department of Housing and Works (Housing and Works) may be able to assist you with different Housing Options based on your current household circumstances.
To find out which options you may be eligible for, complete and submit a Housing Options Assessment (PDF, 2.36MB) (also see download below).
When you submit the assessment, you will need to provide a document that shows your name and, if possible, your date of birth. For example, a Drivers Licence or Medicare Card. If you submit the form by mail, be sure you only send copies of these documents.
You will only need to provide evidence of your current circumstances and income if you decide to submit a formal application.
You can download and complete a Housing Options Assessment (PDF, 2.36MB) and submit it via email, fax or post to your nearest Housing office. If you would prefer, you can visit or telephone your nearest office for help to complete the form.
Find your nearest housing office
Once completed you will be provided with a letter outlining information regarding your potential eligibility (see Rental policy manual (PDF, 1.21MB)) for housing assistance products, as well as their availability, accessibility, and affordability. It will also provide information relevant to your household, history and identified priority need.
The assessment is not an application for assistance and does not guarantee eligibility. On completion of the assessment, you will still be required to complete a formal application for any products you wish to apply for.
