Apply for a Police Information Certificate

A Police Information Certificate (PIC) is only issued in relation to Violence Restraining Orders (VRO) or Family Violence Restraining Orders (FVRO), reaching the Final Order Hearing before a court in Western Australia (WA).

You can apply for a PIC by either:

  1. Emailing interagencyaccess@police.wa.gov.au with the title: Application for Police Information Certificate to submit your request. 
  2. Completing a printed FVRO form (PDF, 185KB) or VRO form (PDF, 184KB) and posting to:
    Interagency Access
    Locked Bag 20
    Perth Business Centre WA 6849

PICs are provided free of charge.
