The approved provider Maragon Australia Pty Ltd, who operates the service, Busy Bees Mirrabooka, was ordered to pay a penalty of $16,000 for contravening section 165(1) of the Education and Care Services National Law (WA) Act 2012 for the offence of inadequate supervision of a child.
A Department of Communities (Communities) investigation found that a child was left unsupervised for approximately 23 minutes inside Busy Bees Mirrabooka, which had closed at the end of the day.
Maragon Australia Pty Ltd was also ordered to pay $1,500 towards Communities legal costs.
