Requirements
Before submitting your completed application form, please ensure you have:
- Completed your name, address and contact details on page 2.
- Signed the statutory declaration on page 5 and had this witnessed (as per the list of authorised witnesses on page 5).
- Provided a certified copy of your photo ID. See How to certify photo ID (PDF, 214.45KB)
- Included copies of any certificates confirming your relationship to the adopted person (if you are a sibling, descendant or other birth relative).
