The Department of Local Government, Industry Regulation and Safety (LGIRS) collects personal information for the purpose of:
- regulating health and safety of workers, at workplaces;
- regulating risks to health and safety arising from work and for related purposes;
- processing your application and verifying your eligibility;
- maintaining licence and registration records;
- undertaking compliance and enforcement activities;
- engagement and educational activities;
- other regulatory functions.
Your personal information is collected under the relevant provisions of the Work Health and Safety Act 2020.
If you do not provide the requested information, LGIRS may be unable to process your application or provide the relevant licence or service.
Your information may be included in an online licence search, published in a register, or disclosed where authorised or required by law. This may include to other government agencies, regulatory bodies, medical or technical experts, employers, trainers, and other third parties.

Under WHS Regulation 143, a licence holder must inform of any changes of information in relation to the licence within 14 days after the licence holder becomes aware of the change, this includes the replacement of a nominated supervisor.
Before you apply
Step 1:
For the proposed nominated supervisor you will need to provide:
- Proof of identification
- Evidence of competency of the below units issued by a Registered Training Organisation (RTO). Equivalent replacement units may also be accepted.
- CPCCWHS2001 — Apply WHS requirements, policies and procedures
- CPCCCM2009 — Carry out basic demolition
- Recent demolition experience relevant to the licence class being applied for
- Experience must be a minimum of three (3) substantial jobs within the past 5 years, in the class for which you are applying. Note: Demolition jobs will be referenced against Demolition of Work Notifications submitted to the WorkSafe Commissioner under regulation 142F.
- If the Nominated Supervisor for this application has been approved as a Nominated Supervisor for a demolition licence holder, within the past 3 years in another jurisdiction under an equivalent legislative framework, or has already been approved as a Nominated Supervisor for another demolition licence holder in WA, within the last 3 years, applicants must provide evidence of this. As such, the evidence provided will be assessed against the requirements and may be considered as an alternative to the SOE template.
- Two written references by two separate referees confirming the experience listed on the Statement of Experience.
- Referees should:
- Have direct knowledge of the work completed, be independent and credible and be able to verify project details and responsibilities provided in the nominated supervisor experience
- Referees need to:
- explain of the referee’s relationship and the capacity to independently verify and endorse the nominated supervisor’s experience
- the type of work completed by the nominated supervisor i.e. occupation, duties, roles, and responsibilities;
- the start and finish date of each project the referee is verifying;
- verify the address of each demolition project listed on the Statement of Experience;
- Full name, email address, and telephone number of the referee.
- Sign and date the reference.
- Referees may be contacted by Licensing Services to verify your experience.
