As part of the application and renewal for an Energy Assistance Payment, Dependent Child Rebate and/or Air Conditioning Rebate from RevenueWA, the concession card holder must authorise us to contact Services Australia to check their customer details and card status.
If you are the concession card holder but you are not the applicant, you must complete this form to enable us to confirm your details.
The applicant must provide a signed copy of this form with the application or renewal, or upload it to a web enquiry.
