To order your replacement plates complete the form and submit it at a Driver and Vehicle Services Centre or regional agent or post it with a cheque or money order to:

It takes about three to four weeks for plates to be remade and you will be notified when they are ready to collect. Your damaged plates must be returned and you will need proof of identification (PDF).
