The Homeless Engagement Assessment Response Team (HEART) is a rapid, agile and intensive assertive outreach service that supports people experiencing homelessness and rough sleeping in the Perth metropolitan region.

Department of Communities (Communities) is commissioning HEART outreach services and is seeking responses from Community Service organisations to register their interest in delivering the successful model.

Once the ROI is complete Communities will commence consultation and engagement to support the implementation of new service agreements set to commence in late 2025.

HEART outreach service providers will continue to play a key role in existing Rough Sleeping Coordination Groups aimed at strengthening coordination and responses to people experiencing homelessness, with a key focus on rough sleeping.

HEART was launched in 2021 as an immediate response to the increasing prevalence people experiencing homelessness and sleeping rough in specific locations.
