To apply for a certified marriage certificate you need to:
- complete and sign the application form
- provide certified copies of relevant identification
- attach any supporting documents
- complete the payment details section
- post all items to:

Payment can be made using MasterCard, Visa, cheque or money order. Do not send original documents or identification in the mail.
Applications for new marriage certificates cannot be made by phone, fax, email, online or in person.
Certificates for previously registered marriages are available by following the same process and posting the application but you can also email it to email@example.com.
