This facility is not for reports where there is no motor vehicle involved.
You will be asked for the following information so please collect it where known:
- date, time and location of crash
- licence plate details of vehicles involved
- circumstances of the crash
- names, addresses and contact numbers of all people involved in the crash and any witnesses
